Mount Sinai Hospital’s Home Care Palliative Program was created in 2001 to offer patients the opportunity to remain at home and receive palliative care. Patients have the opportunity for admission to the Palliative Care Unit should their home situation become too difficult to manage.
Patients are referred to Mount Sinai’s Home Care Program by various hospitals in metropolitan Montreal. To be eligible to participate in the program, patients must reside in the territory of the CIUSSS West-Central Montreal, preferably in the territory of the CLSC René Cassin-Cavendish (Cote St. Luc, Montreal West, Notre-Dame-de-Grace, Hampstead, Snowdon).
Mount Sinai Hospital Montreal offers medical and nursing care in addition to the services offered by the CLSC, which has the mandate to offer home care to the population in its territory.
The main goals of Mount Sinai’s Home Care Program include:
